Enter the name of shareholder and the date of valuation (normally the date of death) in the boxes below, then click Next Page to continue. You will be asked to enter the security details on the next page.
2. Security Information
For each security enter the full security description, currency and SEDOL code
if known (for example, BT Group PLC Ord 5p 3091357) in the Security Name box,
then enter the number of shares/units held or nominal amount in the Holding
Quantity box.
Enter each security held within an ISA or PEP separately with the word "ISA" or
"PEP" after the security description.
Enter cash holdings with the word "Cash" followed by the currency in the
Security Name box, with the amount in the Holding Quantity box.
Click Next Page to continue. The price will be confirmed on the following page.
3. Order Summary
Please check the price and confirm your preferred delivery method below. Click
Submit to continue. We will confirm your details and request your payment on
